Enrollment FAQ

Residency Affidavit Form: For families living with friends or relatives who own/rent in the district

Temporary Residence Waiver: For families purchasing or building a home in Gahanna

Change of Address Form: For families who move within the school district

Franklin County Probate Court Information: For families seeking assistance with custody, guardianship, or power of attorney

Caretaker Authorization Affidavit: For Grandparents seeking consent to all school-related matters for the child 
**Must be notarized and filed with juvenile court within 5 days of execution**

Grandparent Power of Attorney: For Grandparents who have been granted power of attorney of the child, by the parent 
**Must be notarized and filed with juvenile court within 5 days of execution**

Guardianship/Parental Rights Probate Court Form: For adults applying to be granted parental rights via court order

Gahanna Form for Enrolling While Seeking Guardianship: For adults enrolling a student while awaiting the court order granting guardianship/parental rights

Enrollment of Students Over 18 Form:  For students at least 18 but less than 22 years of age who live apart from their parents, support themselves by their own labor, and have not been granted a high school diploma

Ohio Immunization Requirements for School Attendance

Tuberculosis Testing: For any child who has lived in or visited any country outside of the United States for more than 90 consecutive days 

Ordering a New Birth Certificate: For families in need of their child’s birth certificate. Applies to children born in Ohio.

In lieu of a birth certificate you may provide: (1) A passport; (2) An attested transcript of the certificate of birth; (3) An attested transcript of the certificate of baptism or other religious record showing the date and place of birth; (4) An attested transcript of a hospital record showing the date and place of birth of the child; (5) a birth affidavit [O.R.C. 3313.672(A)].

• DOCUMENTATION OF RESIDENCY VERIFICATION

Applicable to:

• Families currently residing in Gahanna
• Families living with friends or family members who reside in Gahanna
• Families moving to Gahanna within 90 days

Documents that are accepted to prove residency include: 

• Real estate tax duplicate

• Mortgage statement along with a current gas or electric bill 

• Current lease along with a current gas or electric bill

Documents must include the parent/guardian’s name and address. An apartment or house lease must list the parent/guardian and student(s) as occupants.

• Residential Parent

• Grandparent Caretakers 

• Guardianship

• Paternity Acknowledgement (unmarried parents)

Your students' bus route for the 2023-24 school year is available on  Transfinder. This web-based service provides Gahanna-Jefferson families 24/7 flexibility and access to route information based on their home address!

Be sure to enter only the street number and street name. Do not enter the zip code or grades or school. (Entering more than the street number and name may cause inaccurate information to display.)

When you receive your bus information, please check over it carefully. If something does not make sense please contact transportation at (614) 751-7581.

Families who move within the school district will need to complete a Change of Address form and provide proof of residency in Gahanna. 

Documents that are accepted to prove residency include:

These items can be brought to the Welcome Center, emailed or faxed. 

Change of Address Form

Preschool enrollment for the 2024-25 school year will open on February 5, 2024, at 8:00 a.m. 

GJPS offers an outstanding preschool program that serves children with disabilities and also provides an educational opportunity for typically developing children to serve as social and academic peer models. Peers MUST be 3.5 years old by August 1, 2024.  If your child is younger than 3.5 by August 1, 2024- please contact Missy Grimmett at grimmettm@gjps.org to discuss enrollment options further. Enrollment is open to children of District residents and staff members of GJPS through an application process.
